Code Integrity: Attempted to load a binary that did not meet code integrity policy requirements

Windows Defender Application Control (`WDAC` / Code Integrity) blocked or audited an unsigned/unapproved driver or DLL.

Event ID 307 (`Microsoft-Windows-CodeIntegrity/Operational`) triggers when Windows Kernel Code Integrity or WDAC blocks (`Enforce`) or logs (`Audit`) an attempt to load an executable, DLL, or kernel driver that fails code integrity verification (`File Name`, `Hash`, `Signer`). Primary telemetry for catching Bring Your Own Vulnerable Driver (`BYOVD`) attacks and rootkit loading.

Failure Reason / Root Cause:

Driver/DLL failed digital signature or WDAC policy check (`FileName` & `SHA256 Hash`).
